Does the form open in Adobe Acrobat? If not you should download that first from here - http://get.adobe.com/reader/ and then download the form here - http://campaign.odw.sony-europe.com/cross/H164/pdf/VAT-back_claim_form.pdf

Are you able to print other pages?

You could always ask at your local store and see if they could print you off a page.
